Fatigue Score Card System

Does your site use a fatigue score card system to determine fatigue levels prior to a shift? If so please share details about the system.


• We drafted a trial for this program but have not implemented it yet. You can use my name as a contact person for the member company to reach out to for more information. However, because our program is in draft form, its not ready to be shared. Still a few kinks to work out.
• <Company> doesn't have a fatigue score card in place although we are investigating different solutions like fatigue science for example.
• We do not conduct any assessment prior to shift using a fatigue score card system to determine fatigue levels prior to a shift.
• Our team does use the fatigue score program. It is part of our fit for duty and it is an expectations that every employee does an assessment every morning prior to heading in to the field. All of our workforce is given a plasticized card to carry with them as your fatigue can change throughout the day. Please see the attached card to what the expectation is. We also give our FLS a training package and provide information on “Getting a good night sleep”
• We do not use a fatigue score card system. Before each shift employees are evaluated based on “fit for work” by the supervisor. Culturally, employees are also supported and encouraged to freely report fatigue issues to their leaders without judgement.
• We have DSS installed on all of our trucks that recognizes fatigue events and interacts with the operator in real time, and we have a self-reporting form that all operators can fill out and submit if they believe they are or will be fatigued during their shift
