Dual Wheel Separation

We have experienced recurring service truck dual wheel separation events, with contributing factors such as missing or ineffective wheel indicators, elongated or “wallowed out” wheel hub and lug holes, and challenges ensuring retorque and maintenance requirements are consistently completed and verified.   • We only have 3 of our own service trucks and they have hardly been used, so no we have not seen it. It is common to see the problem, 9 out of 10 time its after tires have been removed for flat, brake job etc and the mating services are not cleaned probably, or dirt is not between installation. They torque up and then become loose once the grime etc embedded itself. That is why everyone now days puts retorque tags in vehicles.
